Overview

Treadmill Testing, commonly known as Treadmill Test (TMT) or Exercise Stress Test, is a non-invasive diagnostic procedure used to assess the heart’s response to physical stress. It evaluates cardiac function, detects coronary artery disease, and helps identify exercise-induced arrhythmias, ischemia, or abnormal blood pressure responses. TMT is widely used in cardiology for risk stratification, treatment planning, and monitoring of heart disease patients.

During the test, the patient walks or runs on a treadmill while connected to an electrocardiogram (ECG) machine that continuously monitors heart rate, rhythm, and electrical activity. Blood pressure is measured at regular intervals, and the workload is gradually increased according to standardised protocols, such as the Bruce protocol. The patient’s symptoms, exercise tolerance, and ECG changes are recorded to evaluate the heart’s functional capacity and detect any evidence of reduced blood flow to the heart muscle.

Treadmill Testing is particularly valuable in diagnosing angina, assessing the severity of coronary artery disease, evaluating the effectiveness of cardiac treatments or interventions, and determining safe levels of physical activity for patients. It is also used for preoperative cardiac assessment and in preventive cardiology to identify individuals at risk of heart disease before symptoms appear.

The procedure is generally safe, performed under the close supervision of cardiologists and trained staff, with emergency equipment readily available. Patients are advised to wear comfortable clothing, avoid heavy meals, caffeine, and certain medications before the test, depending on medical guidance. Post-test monitoring ensures patient stability and safety. By providing real-time assessment of the heart under controlled physical stress, Treadmill Testing helps detect ischemic changes early, guides therapeutic decisions, and improves long-term cardiovascular outcomes. It is a simple, effective, and essential tool for evaluating cardiac health and planning individualised treatment strategies.
